The Performer has presented the following government-issued photo identification to Tsan Motion Picture Ltd. prior to filming. A legible copy is attached to this Agreement and retained by the Custodian of Records.

演出者已於拍攝前向 Tsan Motion Picture Ltd. 出示下列政府核發附照片身分證明文件。清晰影本附於本協議並由記錄保管人留存。

Section IV — Age and Consent Declarations / 第四節：年齡及同意聲明

The Performer declares and represents, under penalty of perjury pursuant to 28 U.S.C. § 1746, that:

演出者依 28 U.S.C. § 1746 偽證罪罰則，聲明並保證：

1. Age / 年齡： I was born on the date stated in Section I. I was at least eighteen (18) years of age at the time all visual depictions described in Section III were created. I understand that the production of sexually explicit material involving a person under 18 years of age is a serious federal crime under 18 U.S.C. § 2256 et seq. The information I have provided regarding my age and identity is true and correct.
   本人出生日期如第一節所載。本人在第三節所述所有視覺描繪內容製作時，年滿十八（18）歲。本人了解製作涉及未滿 18 歲人員之性露骨內容，依聯邦法 18 U.S.C. § 2256 等相關規定為嚴重刑事犯罪。本人所提供之年齡及身分資訊均屬真實正確。
2. Voluntary Participation / 自願參與： I freely and voluntarily consent to participate in the production described in Section III. I have not been subjected to any threat, coercion, deception, debt bondage, or undue influence, and I have had a reasonable opportunity to review this Agreement before signing.
   本人係自由、自願同意參與第三節所述製作。本人未受任何威脅、脅迫、欺騙、債務控制或不當影響，且簽署前已有合理機會審閱本協議。
3. Consent to Filming / 同意拍攝： I consent to being filmed, photographed, and recorded in any audiovisual format in connection with the production described in Section III, including the performance of simulated and/or actual sexually explicit acts as agreed with the production company prior to filming.
   本人同意在第三節所述製作中以任何影音格式被拍攝、攝影及錄製，包含拍攝前與製作公司協議之模擬及／或實際性行為。
4. Consent to Distribution and Publication / 同意發行與公開散布： I grant to Tsan Motion Picture Ltd. and AN9 L.L.C. the irrevocable (subject to Section V below), worldwide, perpetual right and license to use, distribute, publish, display, transmit, and sub-license the content in which I appear, including on the platform an9tv.com and in any derivative or promotional works related thereto.
   本人授予 Tsan Motion Picture Ltd. 及 AN9 L.L.C. 不可撤回（受以下第五節限制）之全球性、永久性權利及授權，得使用、發行、公開、展示、傳輸及再授權本人出演之內容，包含在 an9tv.com 平台及相關衍生或宣傳作品中使用。
5. Consent to Download (if applicable) / 同意下載（如適用）： □ I also consent to the content being made available for download by subscribers of an9tv.com.   □ I do NOT consent to downloadable distribution.
   □ 本人亦同意該內容提供給 an9tv.com 訂閱用戶下載。   □ 本人不同意提供下載。

Section V — Rights Assignment and Work-for-Hire / 第五節：權利讓與及職務著作

The Performer acknowledges and agrees that:

演出者確認並同意：

1. All audiovisual content produced under this Agreement is produced as a work made for hire (as defined under 17 U.S.C. § 101) on behalf of Tsan Motion Picture Ltd. and AN9 L.L.C. To the extent any work is not deemed a work made for hire by operation of law, the Performer hereby irrevocably assigns all right, title, and interest in such work — including all copyright and related rights — to Tsan Motion Picture Ltd. and AN9 L.L.C.
   依本協議製作之所有影音內容均為受僱作品（依 17 U.S.C. § 101 之定義），歸屬於 Tsan Motion Picture Ltd. 及 AN9 L.L.C.。如任何作品依法未被認定為受僱作品，演出者茲不可撤回地將該作品之全部權利、所有權及利益（包含著作權及相關權利）讓與 Tsan Motion Picture Ltd. 及 AN9 L.L.C.。
2. The Performer retains no ownership rights in the content produced. The Performer's compensation, if any, is as separately agreed in the applicable engagement agreement and does not confer any additional rights in the content.
   演出者對所製作之內容不保留任何所有權。演出者之酬勞（如有）依另行簽訂之工作合約約定，並不賦予對內容之額外權利。
3. Notwithstanding the foregoing, the Performer retains the right to request removal of content at any time pursuant to the Content Removal & Takedown Policy of an9tv.com. Such a request does not constitute reclamation of copyright ownership.
   儘管如上所述，演出者仍保留依 an9tv.com 內容下架與撤除政策隨時申請移除內容之權利。該申請不構成著作權所有權之取回。

Section VI — 18 U.S.C. § 2257 Compliance Declaration / 第六節：18 U.S.C. § 2257 合規聲明

The Performer acknowledges that:

演出者確認：

1. Tsan Motion Picture Ltd. is the primary producer (as defined in 28 C.F.R. § 75.1(c)(1)) of all content produced under this Agreement and is responsible for maintaining the records required by 18 U.S.C. § 2257 and 28 C.F.R. Part 75.
   Tsan Motion Picture Ltd. 為依本協議製作之所有內容的主要製作方（如 28 C.F.R. § 75.1(c)(1) 所定義），並負責維持 18 U.S.C. § 2257 及 28 C.F.R. Part 75 所要求之記錄。
2. The records required to be maintained, including a copy of this Agreement and a copy of the Performer's government-issued photo identification, are kept by the Custodian of Records:
   應維持之記錄（包含本協議副本及演出者政府核發附照片身分證明文件副本）由以下記錄保管人保管：

3. These records are available for inspection by the Attorney General of the United States, or a designated representative, as required by 18 U.S.C. § 2257(c). They will not be disclosed to any other party except as required by law. Full compliance details are published at an9tv.com/2257.html.

Section VII — Signatures / 第七節：簽名

By signing below, the Performer confirms that they have read, understood, and freely agreed to all terms of this Agreement, that all information provided is true and accurate, and that they execute this Agreement under penalty of perjury pursuant to 28 U.S.C. § 1746.

演出者於下方簽名，確認已閱讀、理解並自願同意本協議所有條款，所提供資訊均屬真實正確，並依 28 U.S.C. § 1746 偽證罪罰則簽署本協議。
